    Does anyone have a policy on Telemetry strip documentation in the ICU? Please share if you do.

    On admission, during any significant event, a strip is ran every 4 hours, we have arrhythmia record we tape it to. Must be interpreted with a signature. Strips also print out alarm limits and if they are on.

    If the patient goes in and out of A-Fib all night long you do not need a strip for each change.
    BUT, if the patient goes in and out of V-TAC. need a start of the rhythm and a strip when goes out. Out EP dock likes to see that.
