    Wound Assessment Form

    Hello. I just took over the wound care at a transitional care center, and I hate the forms they currently use to monitor wounds. Does anybody have a copy of a form I could take a look at or a link to a website with a few. Thanks!

    Mark, I'd like to see the ones you use. Figure nothing could be as bad as the ones I used. Maybe we can come up with a better one? If not, will see what I can find.....

    I currently don't have access to the ones we use now. Basically, there are two forms we use now for wounds. One is a pressure-related sheet with a place to circle the area affected. Underneath is where you stage the wound, and describe undermining, drainage, color, etc. There is also a small area to put the current treatment. Our other sheet is used for anything else that is not pressure related (surgical, skin tears, diabetic, and so on). There is also a place to put comments on these as well. I really want a sheet that combines these so we can decrease the amount of paperwork. Thanks for the help!

    I am the wound care nurse for a facility that is very rigid with their policies in that we cannot determine what forms are used, it is a corporate decision (and I'd swear that if they actually had to use the forms themselves they'd never have picked them) but I suppose they aren't terrible. The skin condition record form is by Briggs:

    I think the one I have to use is a corporate made form though.

    Will do. We use Briggs also, but I use other forms for my OWN use.

    I actually came up with my own form with a little help from some other forms I found. I tried to upload it on here but the file size is too large. If anybody needs it for reference let me know and I can email it to you. Thanks for the help!
