    Care plans that make you say WTH?

    I'm sure we've seen them at some time in our careers: the care plan that makes you wonder 'What were they thinking?' :confused:

    Care to share them?

    We have the same problem every time we get a new bunch of residents (so every 6 months!). They don't know how to enter orders and they do not bother to ask...even though they get trained on it! So we have one order for vital signs that reads

    Vitals-
    vitals q4h x4 then qh8, up ad lib, diet npo, hygiene assist, change dressing bid and prn when soiled, ng may come out when flatus, etc... (and sometimes you get orders that don't even apply to the patient!!! Like Ostomy care prn...when they don't have an ostomy!)

    So instead of getting something under the diet section, hygiene and care section, and nursing procedures you have them all under vital signs. (SO YOU HAVE TO GO BEHIND THE "CHILDREN" AND FIX THE CAREPLAN SO NOTHING GETS MISSED!):33:

    Does ambulate as tollerated for a double amputee count as WTH?
    "BECAUSE I SAID SO" IS NOT A GOOD ENOUGH REASON.

    I would say so. Someone was either sleeping on the job, or severely brain damaged IMO.
