Hospice admissions nurse

CA-Claremont, Coordinates services for patients electing the VNA Hospice Program. Communiates with physicians, hospital/facility staff and referral sources regarding patient care and available agency services. Evaluates potential hospice patients and reports findings to the patients attending physician and obtains orders.
