WI-MILWAUKEE, RN HOME TODAY SURGERY - PER DIEM HOURS - COMMUNITY MEMORIAL HOSPITAL Implements individualized plan of care by providing quality patient care to HTS patients from (pre)admission to post discharge in collaboration with other disciplines. Assists patients to meet discharge goals by providing education to meet their needs. Provides individual assessment, support and teaching for the AM admit patients
