CA-San Diego, JOB SUMMARY: The case manager coordinates provision of care for pediatric patients of varying ages and developmental levels. The case manager will advocate for the patient/family by collaboration with the multidisciplinary team to develop and implement the plan of care including discharge plan to ensure quality patient outcome and customer satisfaction.
