Nurse Practitioner

IL-Champaign/Urbana, Matrix Medical Network is a national physician practice that contracts with health plans to complete health risk assessments on plan members in a home setting and/or Skilled Nursing Facility. We are currently seeking a full-time nurse practitioner to complete health assessments in homes and Skilled Nursing Facilities throughout the greater Champaign/Urbana area.
