SC-Charleston, RN Case Manager with hospice experience and minimum of one year acute care exp. required. RN Case Manager provides skilled nursing care to hospice patients and oversees delivery of hospice nurse aide services. Care provided to patients in SNF's, ALF's and home settings. Candidate must be energetic, compassionate, customer service oriented, and self-motivated
