NY-New York City, Reporting to the RN Team Manager, this position will provide homecare services to patients on our Hospice Program in their home in the Bronx. You will assess, plan, implement and evaluate delivery of care provided on an ongoing basis. You will provide teaching to our patients and family/caregiver as needed. You will document homecare services provided electronically and make any changes as needed.
