Licensed Practical Nurse (LPN) - Continuous Care

TX-Dallas, The Licensed Practical Nurse is responsible for coordinating the patient’s and family’s care. Provides skilled nursing care as determined by the interdisciplinary plan of care. Teaches families and other primary care persons appropriate care techniques. Provides accurate documentation with visit itineraries. This is a PRN position with some flexibility in work shifts. Hours are EXTENDED NIGHT SHIF
