Wanting to change nursing fields-resume?

I am a RN that has 14 years geriatric experience in the nursing home environment. The pressure to take care of so many pts. is getting to me, not only emotionaly(who has time to really care) but I also fear for my license @ times. So I want to transition into something like homehealth,probably for the elderly due to that is what I have grown to really enjoy and use my past expriences. however, I am @ a loss as how to prepare a resume for this transition.

I recently did a new resume as formats have changed
I googled medical resumes and picked the one I liked the best, printed it and followed the outline, it was easy. It will show you how to make a clear concise opening statement as to what it is you want employers to see.
The home care company will still have you fill out an application.
