Nurse Navigator - Spine Program, RN

AZ-Phoenix, The Nurse Navigator – Spine Program, RN is responsible for coordinating the care of services for patients of the Spine Program, providing outreach to referral sources, and developing relationships with non-JCL outgoing referral resources. This includes, but is not limited to, assuring appropriate communication and follow-up and at each point of care, maintaining relationships with referral network
