Nurse rn case manager

, Nurse-RN Case Manager NYLicensed Home Care Agency seeks person to coordinate overall interdisciplinary plan of care for a group of patients from admission to discharge. Will act as a liason between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. Must be a registered nurse with 2 - 4 years of clinical exp. and be familiar with standard concepts
