    nursing diagnosis...plz help

    hey..
    i need ur help very urgent...

    and seek for ur replies as soon as possile...!!

    i need examples of nursing diagnosis...!!!


    for the five types of nursing diagnosis...
    actual, risk, wellness, possible and syndrome

    each type i need to find 2 examples...!!

    am student...and "FRESHMAN"
    GOD...!!
    i dnt know wats these thing yet...!!

    plzz i need them soooo badly..!!!

    heeeeeeeeeeeeeelp.........!!!

    If you can get to a good bookstore look for a book called the Nurses Pocket Guide to Diagnosis, treatment plans and Rationales. I carried it all through nursing school and it saved my bacon more than once.

    If you go on Amazon.com, you might find some e-books that you can gain access to immediately. I hate hate hate Nsg diagnosis. I think it is on it's way out. At UCSF, in my master's program, we didn't even mess with it. We believe we are just as able to use medical dx as any other medical professional

    What the heck! Said post moved to Student Nurses Forum, just because.

    Welcome to the site, aly! I hope you stop by and ask any and all questions you may have. We've all been freshmen at one time. TRUST me.

    I think Nursing Dx's are gonna stick around. Sorry, Syck! Personally, I think the Dx's will help us all think about how a particular disease impacts the patient. Aly, does this make sense to you?

    Probably has one of Syck's links in there. I can't find my book for the life of me. However......here are some examples, using the NANDA list: http://www.efn.org/~nurses/nanda.html

    Say your patient has CHF.

    What is the ACTUAL dx? Heart failure. Look at the aforementioned list. Circulation (actual dx) would be a good bet, with decreased cardiac output.

    Is the CHF newly diagnosed? Then the patient may also have a nursing diagnosis of Knowledge Deficit: they need to learn some things from us----medication they will be taking, side affects, WEIGHT GAIN! If more than 'x' number of pounds in so many days, let health care providers know......

    Possible: Is there activity intolerence? CHFer's can become out of breath easily, due to poor circulation/fluid build up.

    Wellness? ABSENCE of any CHF symptoms.
