    Care plans
    The statements below refer to care plans. Are they true or false?
    1 . A care plan is a ‘do-to’ list.
    Answer FALSE: A 'to-do' list simply enumerates the tasks you should do, a care plan also includes the steps you need to perform the task and the challenges you may face to do it.

    2 . The language used in care plans should be straightforward and simple.
    Answer TRUE: The language you use to write a care plan should be kept simple so that everyone reading it is able to understand what action is needed to complete the set tasks.

    3 .Care plans start by listing the client’s problem/s.
    Answer FALSE: The first step in writing a care plan normally involves assessing the patient’s needs on admission.

    4 . Goals set in a care plan should be realistic.
    Answer TRUE: There are five rules that the goals in a care plan should observe: to be realistic is one of them.

    5 . The care provider should re-evaluate the care plan to adjust it to the patient’s changed needs.
    Answer TRUE: There are five steps in designing a care plan; re-evaluation of the care is the last one.

    Before I hate writing care plans as we are required by our CI’s to do it handwritten, so I literally have to spend a lot of time just to finish one copy and then pass it as a requirement! Now we are already allowed to print it and this link is really a good resource and will make my student life a lot easier!
