    Nclex question of the day

    The nurse is caring for a patient with a chest tube. Which of the following observations requires an immediate intervention by the nurse?

    1. The suction regulator is set at 100 mm Hg
    2. Drainage in the collection chamber is greater than 100 ml/hr
    3. Constant bubbling in the suction control chamber
    4. Tidaling in the water seal drainage
