Eyeball it. There aren't too many sizes to choose from and should be able to guess it right.
Usually the size of the pad you choose will depend on the level of incontinence for that particular person, since different sizes pads will hold different amounts of urine. Generally, you wouldn't use a massive pad on someone who wasn't overly incontinent. A proper assessment from a continence nurse is often the best way to establish the right pad for the right patient, especially in a nursing home.
Obviously the patients overall size/build does come into it a little but most of the good brands fit most people unless they are really big.
I eyeball it we have two choices ...large and larger lol. Wish we had bariatric briefs but o wellie
